Meldic Health Clinic offers integrated primary care and mental health services including but not limited to:

- Mental Health Therapy & Psychiatric Services
- Telehealth Consultations
- Substance Use-Related Medical Care
- Care Coordination & Specialist Referrals
All services are provided by licensed healthcare professionals. Our services do not constitute an emergency medical service. In case of a medical emergency, please call 911 immediately.
3. Appointments & Cancellations
Patients are required to schedule appointments in advance through our website or by phone. We ask that you provide at least 24 hours notice if you need to cancel or reschedule your appointment.
Repeated no-shows or late cancellations may result in a cancellation fee or discharge from the practice, at the discretion of the clinic.
4. Telehealth Services
Telehealth services are offered as a convenient alternative to in-person visits where clinically appropriate. By using telehealth:
- You consent to receiving healthcare services via electronic communication.
- You acknowledge that telehealth has inherent limitations compared to in-person care.
- You are responsible for ensuring a private, secure environment during your session.
- You confirm that you are located in a state where our providers are licensed to practice.
5. Patient Responsibilities
As a patient of Meldic Health Clinic, you agree to:

- Pay all applicable fees, co-pays, and balances in a timely manner.
6. Payment & Insurance
Meldic Health Clinic accepts most major insurance plans as well as self-pay patients. You are responsible for verifying your insurance coverage prior to your appointment. Any balances not covered by insurance are the patient’s responsibility.
We reserve the right to require payment at the time of service. Unpaid balances may be referred to a collections agency after reasonable notice.
